Dog Training Waiver

I. Introduction

This Dog Training Waiver Agreement ("Agreement") is entered into between LIFETRACKS TRAINING (referred to as "Trainer") and the undersigned pet owner ("Client"). This document outlines the terms under which dog training services are offered by LIFETRACKS TRAINING and accepted by the Client. The training will take place at LIFETRACKS TRAINING or other agreed upon locations.

II. Acknowledgment of Risk

• Client acknowledges that dog training entails inherent risks.

• Risks include injuries to animals, people, and property.

• Risks may arise from canine behavior beyond the Trainer's direct control.

• The Client agrees to assume all risks associated with training sessions.

• This acknowledgment ensures that the Client understands and accepts the potential risks involved in dog training.

III. Waiver of Liability

• Acknowledgment of Risk: The Client acknowledges the potential risks associated with dog training, including injuries to animals, people, and property arising from canine behavior outside the Trainer's direct control.

• Assumption of Risk: The Client agrees to assume all risks associated with participating in the training sessions.

• Release and Indemnification: By signing the Agreement, the Client releases, indemnifies, and holds harmless LIFETRACKS TRAINING, its agents, successors, assigns, and employees from any claims, damages, lawsuits, losses, liabilities, and expenses related to the dog training services provided.

• Limitation of Liability: The waiver does not apply to injuries or damages caused by gross negligence or intentional misconduct by the Trainer.

• Enforceability: The Client acknowledges that the terms of the Agreement are legally binding upon signing and that they have read and understood its contents.

IV. Veterinary Care

The Client confirms that the dog participating in the training sessions is in good health and has been vaccinated according to state and local requirements. In the event of an injury or illness, the Client authorizes LIFETRACKS TRAINING to seek emergency veterinary treatment. Costs for such treatment will be the responsibility of the Client.

V. Training Methods

LIFETRACKS TRAINING uses positive reinforcement techniques in training. By signing this Agreement, the Client consents to the use of such methods with their dog and acknowledges that results can vary. The Trainer will make efforts to achieve the desired outcomes but cannot guarantee specific results from the training program.

VI. Payment and Cancellation Policy

The Client agrees to adhere to the published rates, payment schedules, and cancellation policies of LIFETRACKS TRAINING. Payments are due at the time of registration unless otherwise arranged. Cancellations must be made at least 24 hours in advance to avoid penalty charges.

VII. Photo and Video Release

During training sessions, the Trainer may capture photos and videos of the dog for educational and promotional purposes. The Client grants LIFETRACKS TRAINING permission to use such images on its website, in advertisements, and on social media platforms.

VIII. Agreement Duration and Modifications

This Agreement is effective from the date of the Client's signature and remains in effect for 6 months.

By signing below, the Client and Trainer agree to the terms and conditions of this Agreement and affirm that the information provided is accurate.
